Cornerstone Early Learning Center Enrollment Procedures

Welcome!  We are excited to have you become a member of our Cornerstone family. 

In order to complete the enrollment process, the following forms must be completed and on file with Cornerstone Early Learning Center, Inc. prior to the first day of attendance:

• Enrollment Form (usually submitted at time of enrollment)
  
• Consent Form
  
• I am Special
  
• DHS Medical Form (This form is provided by your Doctor)
  
• Copy of Official Birth Certificate
  
• Late Pick-Up Policy
  
• Behavioral Support and Transition Policy
• DCFS Verification Form (please sign verification on the last page)
  
• Pest Management Plan
